In order to complete the application to enter into the Fundamentals of Psychosynthesis you may do so either online (using this form), or by downloading the pdf version of the application form, printing it off and sending it by hard copy to the Institute.

 

Notes:

When filling out the questionnaire section type your response into the box under the question. You are not restricted by the number of words you can type into this box however, if your response is lengthy some of the text may scroll out of view. This can be recovered using the scroll bars on the righthand side of the response area. All of the information you enter, whether visable or not will be sent in its entirety to the Institute once you click the submit button at the end of the questionnaire.

 

All questions require a response and should be completed as fully as possible. In the event that you have no response to a particular question you must enter n/a.

With your submission of this application /questionnaire, a cheque for the non-returnable deposit of £50.00 should be forwarded to the Institute for the Fundamentals Course and a further £50 for either the CBT+ or Introduction to Coaching courses. (See address at the foot of the page)


Cheques should be made payable to The Institute of Psychosynthesis.
